PHYSICIANS CREDENTIALS TAB


The Physician's Credentials Tab allows you to add the physician's license information as well as any identifiers for the physician such as:

  • Drug Enforcement Administration
  • National Provider Identifier
  • Tax Identification Number
  • Taxonomy Code

ADD LICENSE  (Top)

To add the physician's license number, click on "Add License". 

NOTE: For Careficient Agency Administrators, click on the following to learn more about the privileges for Adding Referral Sources.

Choose the "State" from the drop down.  Enter the required information and click "Save" when finished.

EDIT LICENSE  (Top)

To edit the physician license, click on the "Edit Pencil" icon.

NOTE: For Careficient Agency Administrators, click on the following to learn more about the privileges for Editing Physicians

Make the necessary changes and click "Save".

DELETE LICENSE  (Top)

To delete the physician license, click on the "Trash Can". 

NOTE: For Careficient Agency Administrators, click on the following to learn more about the privileges for Deleting Physicians.

You will be asked if you are sure you want to delete.  Click "OK" to confirm.

ADD IDENTIFIER  (Top)

To add an Identifier, click on the "Add Identifier" link. 

NOTE: For Careficient Agency Administrators, click on the following to learn more about the privileges for Adding Referral Sources.

Choose the "Identifier Type" from the drop down.  Enter the required information and click "Save" when finished. 

EDIT IDENTIFIER  (Top)

To edit the physician's identifier, click on the "Edit Pencil".

Make the necessary changes and click "Save".

DELETE IDENTIFIER  (Top)

To delete the physician's identifier, click on the "Trash Can".

You will be asked if you are sure you want to delete.  Click "OK" to confirm.
