ADD REFERRAL/PATIENT  (Top)

The Referral tab displays information that was gathered during the intake or referral process such as referral source, referral contact and additional referral information.  Depending on the referral source, you may not have complete patient information. EDIT REFERRAL SOURCE  (Top)

To edit the referral source, click on "Edit".  Choose the appropriate Referral Source and click "Save" when finished. Click here to learn more about Adding Referral Source.

EDIT REFERRAL CONTACT  (Top)

To edit the Referral Contact, click on "Edit".  

Choose the Referral Contact.  Click here to learn more about Add Referral Contact.

EDIT REFERRAL INFORMATION  (Top)

To edit Referral Information, click on "Edit".

Make the necessary changes and click "Save" when you are finished. 

EDIT FACE TO FACE REFERRAL INFORMATION  (Top)

To edit "Face to Face Referral Information", click on the Edit link.  Make the necessary changes and click "Save" when you are finished. You can not make any changes to the Physician’s Face-to-Face information on the Referral tab if the 485-Physician Orders that includes the Face-to-Face information has been sent to or received from the Physician. This edit eliminates making changes to this information once it has been distributed to the Physician for signature. If you need to update this information, you will need to change the 485-Physician Orders back to an “approved status”. 

CHANGE FACE TO FACE DOCTOR  (Top)

You can also change the Face to Face Doctor by clicking on the "Change Face To Face Doctor" link.

On the Edit Referral Information section, you will be able to see who created the referral as well as who modified the referral last. 

Modified by – displays the user name of the individual that modified the referral

FACE TO FACE REFERRAL INFORMATION

During the referral entry process, you have the ability to enter the Physican’s face-to-face information.   If you did not, you can enter the face-to-face information on the Patient Encounter-Referral tab. Select "Add Referral" Link

Add/Update the information and select the "Save" button.

Face-to-Face information that you collect at time of referral displays on the 485 (display, print and fax views).   The following statement will print on the 485 above the physician signature section (locator 27):    

"I further certify this patient had a face to face encounter completed on mm/dd/yyyy (based on date from the referral) by xxxxxxx xxxxxx, xxxx (physician first name, last name and title based on information from the referral)".  

NOTE: For Careficient Agency Administrators, you must turn on the rule "Apply Face to Face narrative to Orders from Referral" and also "Apply F2F referral data to 485 F2F document" in order for the statement to appear on the 485.
