HOSPICE PATIENT SURVEY

You can create the patient survey information required for HHCAHPS reporting. To create patient survey information go to Processes > Hospice Patient Survey.

NOTE: For Careficient Agency Administrators, click on the following to learn more about the privileges for Hospice Patient Survey.

The following information displays on the Patient Survey page.  Choose the drop down to find the specific format that your survey vendor requires.

Choose the Month/Year for the export

You can filter by agency...

 

Click "Search" when you are finished with your filters. 

Click on "Export" when you have filtered the report to your specifications. This will create a spreadsheet for you to upload to your surveyor.
