HOME HEALTH QAPI REPORTS
A series of Quality Assessment and Performance Improvement (QAPI) reports are now available to help you implement, evaluate and maintain a QAPI program that involves all Home Health Agency services Outcome reports use data collected from OASIS documentation and Service Notes as well as Patient Encounter information. Information provided is to help you improve outcomes where needed, including use of emergency services as well as hospital admissions/readmissions. These reports allow you to address your Home Health Agency’s performance. including prevention and reduction of medical errors.
Reports that are available include the following:
- Pain Management – Discharge - Compares Start of Care OASIS M1242 to Discharge OASIS M1242; identifies how many patients met the criteria along with total patients evaluated
- Hospitalizations/ER Visits - Reviews Discharge OASIS M2301 records that have a response indicating that the patient was admitted to the ER or hospital; identifies how many patients met the criteria along with total patients evaluated
- Shortness of Breath Management - Discharge - Compares Start of Care OASIS M1400 to Discharge OASIS M1400 to determine if there is a decrease in shortness of Breath; identifies how many patients met the criteria along with total patients evaluated
- Improvement in Ambulation – Discharge - Compares Start of Care OASIS M1860 Care to Discharge OASIS M1860 to determine if patients did not improve in ambulation; identifies how many patients met the criteria along with total patients evaluated
- Improvement in Transfer – Discharge - Compares Start of Care OASIS M1850 to Discharge OASIS M1850 to determine if patients did not improve in transferring; identifies how many patients met the criteria along with total patients evaluated
- Improvement in Bathing - Discharge - Compares Start of Care OASIS M1830 to Discharge OASIS M1830 to determine if patients did not improve in bathing; identifies how many patients met the criteria along with total patients evaluated
- Improvement in Wounds - Evaluates all responses documented on the Discharge OASIS M1313 to determine if there is no improvement in Wounds. If M1313A, M1313B, M1313C, M1313D, M1313E and M1313F have a response of 1 or greater, then the pressure ulcer is worse; identifies how many patients met the criteria along with total patients evaluated
- Timely Admission - Compares Start of Care OASIS M0102 or M0104 to the Patient’s Start of Care date M0030 to determine if your Agency did not admit the patient timely (within 48 hours); identifies how many patients met the criteria along with total patients evaluated
- Patient Assessed for Depression - Reviews responses to Start of Care OASIS M1730 to determine if the patient did or did not have a depression screening; identifies how many patients met the criteria along with total patients evaluated
- Medication Management - Compares the response to OASIS M2020 at time of admission to OASIS M2020 at time of discharge and identifies if medication management has improved, declined or if there is no change
