CLINICAL UPDATE ORDER
A "Clinical Update Order" makes changes to the following 485 locators during an episode when information changes:
- Principal Diagnosis/Date
- Surgical Procedure/Date
- Other Pertinent Diagnosis/Date
- Other Surgical Procedures/Date
- DME and Supplies
- Safety Measures
- Nutritional Requirements
- Allergies
- Functional Limitations
- Activities Permitted
- Information for the most current Clinical Update document will flow into the 485 for the next certification period (if no clinical update form exists, then the information from the current 485 will flow to the next 485)
- Does not impact frequency
Example of Clinical Update Order
You can also use the Clinical Update interim order to document information based on Case Conferences/Meetings you may have in your office to review patient status/ongoing patient care. As part of this new feature, you will also be able to identify if this document is being used as a Case Conference/Meeting or ongoing changes you make to the information during the certification period. The Clinical Update Interim Order has a new section indicating if you are using the form for your Case Conference/meeting or just creating a typical/routine clinical update for ongoing changes you want to keep track of during the certification period (i.e., change in diagnosis information that occurred during the certification period that are not documented on the 485-physician orders; does not include discipline/frequency/ duration information)
Other screens/reports will show that you are using the Clinical Update as documentation for your Case Conference meeting or be flagged as “CC” next to the type of form you created (i.e., Clinical Update vs. Clinical Update-CC for your Case Conference).
