FACE TO FACE DUE REPORT

Face to Face Due Report (Hospice) - Provides a listing of all hospice patients that require a physician’s Face to Face visit based on the certification date range selected.

Select an agency

Enter a date range (uses certification “to” dates as the range; defaults to 30 days prior to 30 days in the future from current date)

Select the "Search" button (information displays on the screen)

REPORTS

Choose the type of report that you want by clicking on the drop down and then click "Export".

Face To Face Due Report (PDF) includes:

  • Patient Name
  • From Date
  • To Date
  • Election Period
  • Effective Date
  • ID
  • Status
  • Physician Name

Face To Face Due Report (Spreadsheet) includes:

  • Agency Name
  • Patient Name
  • Certification From Date
  • Certification To Date
  • Hospice Election Period
  • Effective Date
  • ID
  • Status
  • Physician Name
