PATIENT FACT REPORT LINK

The "Patient Fact Report" will print out a list of the following choices:

• Include Physicians/Facilities
• Include Patient Contacts
• Include Notes
• Include Payers
• Include Clinical
• Include Referral
• Includes Medications (all)
• Includes Medications (active only)
• Includes Orders (all)
• Includes Orders (current only)
• Include Schedules

You can print the Patient Fact Report in either Portrait or Landscape Orientation. 

Make the choices for your report and click "Export".

The report displays on the screen. You can use your browser functions to print/save the report as necessary.  You can also print this report in a portrait or landscape format for all you Patients based on the date range you enter.  Additionally, the AMS Patient Fact report that we automatically create daily and put in your automated reports queue will automatically print in the portrait format.
