ADD INTERDISCIPLINARY REVIEW (HOSPICE)
The Medical Director has to sign the IDT Review as it is considered an order. The agency needs to show that you collaborated with the PCP regarding IDT. Most agencies document that the PCP was collaborated with and then fax the IDT to him/her. The IDT Process is to stay open the next IDT as soon as the current IDT is approved. This IDT stays open until the next IDT date (2weeks) so that all of the IDT comments from the service notes can flow to the IDT Review document. IDT is held, patients are discussed and any discussion is documented on that IDT Review. When IDT is over the IDT is signed and approved and then the doctor can sign it. (don't forget to open the next IDT again). Please be aware if you approve the Care Plan prior to completing the Care Needs, they will not flow forward to the next IDT.
This order will not allow you to put in the wrong effective date. If you try to put in a date that is 2 days after IDT should be scheduled, it will not allow you to do that. This is your Care Plan that will have everything on it that you’ve already added. Under section “21 Comments”, if you don’t already have a process on how you are doing your IDT minutes, this is a good place to do this. When doing IDT, there has to be someone who is taking notes on the discussion that went on about the patient. Surveyors will be able to see the documentation here.
