ACCESSING THE CAREFICIENT FAMILY PORTAL (Top)
The Agency that is providing service to you, a family member and/or a friend will be able to provide access to the family portal using a valid email address. To obtain this access, you can contact the Agency that is providing this service. Once the Agency gives you this access you will receive the following email:
To access the Family Portal, select the link as identified in the email you received. Be sure to save this link as one of your “favorites” for future use! After you select this link, the Careficient sign-in screen displays. Enter the following information:
The Organization (Org) code that appears in the "welcome" email you received
Your user name as identified in the "welcome" email (should be your email address)
Temporary password as identified in the "welcome" email
Select the "Sign In" button
When you first log in, you will be prompted to change your password:
"Password" - enter the password that you were either emailed or your facility administrator provided you with.
"New Password" - enter the password you want to change it to
"Confirm New Password" - enter the password you want to change it to
"Change Password" - click when you have completed the above three steps
** Passwords must be at least 6 characters and contain at least 1 number (s)**
After you sign in you will have access to information for the Patient. Information is organized by tab based on the type of information you want to review. You select a tab on the screen to access any of the following information:
