Special programs the agency may have that have standard protocols the agency caregivers follow when providing patient care (usually only assigned when the patient’s diagnosis and condition are critical and this is a proven standard of care to improve the patient condition and outcome)

Vaccinations the patient has had are identified here.

DISASTER PLANS  (Top)

Disaster plan protocols identified along with plan details in the case of a disaster such as tornado, hurricane, earthquake, etc. (protocol options identified by federal, state and local authorities)

CONSENT AND NOTICES  (Top)

Consent for treatment and other notices identified and signed by the patient (usually when they initially start services) to allow the agency to provide the service, bill payment sources and general information the patient needs to be aware of).
